Residency Application Requirements

Application Deadlines:

PGY 1 Program – January 1, 2018

Critical Care Pharmacy – January 1, 2018

Geriatric Pharmacy – January 5, 2018


Qualifications
  • Doctor of Pharmacy Degree from an ACPE-accredited school of pharmacy
  • Eligible for licensure in the state of West Virginia
  • PGY2 Applicants must have successfully completed a PGY1 Residency in Pharmacy Practice
  • Residency requires a specific set of minimum physical, mental, emotional and social abilities necessary to be successful. Residents must possess all of the abilities listed in the five categories outlined in this policy (See CAMC Staff Handbook D, page 8-10 for detailed discussion of standards).
